Arthur Care is committed to helping people with disabilities to exit hospital settings who are clinically ready to leave the hospital and move into their community. Arthur Care works closely with Hospitals, Allied Health Professionals and Families to make this transition as smooth as possible. Our aim is to connect people with their community, formal, informal and mainstream supports.
Our experienced support team can assist you to liaise with the NDIS by preparing the required / requested assessment to start the transition process. Whilst the ideal accommodation is identified we can arrange a Short-Term Accommodation [STA] or Medium-Term Accommodation [MTA].
